A comprehensive evaluation conducted by a licensed professional to understand your needs, concerns, and goals. This session helps determine the most appropriate treatment plan tailored specifically for you. 

We offer individual counseling tailored to meet the unique needs of each client. Our one-on-one sessions provide a safe, confidential space to support personal growth, healing, and emotional well-being.

Many individuals experience anxiety, fear, and hopelessness following painful or traumatic events. We provide compassionate support to help you navigate grief and healing through individual, family, and group therapy services.

Adult and minor clients work one-on-one with a trained mental health clinician in a safe, caring, and confidential environment. Our counselors provide empathetic, nonjudgmental support, creating a space where clients can openly explore their thoughts, feelings, and experiences.

Group therapy provides adults and minors with a supportive space to connect with others, reduce feelings of isolation, and learn healthy coping and communication skills. Participants benefit from shared experiences, peer support, and guidance from licensed clinicians in a safe, structured environment that promotes emotional growth and resilience.

Our psychiatrists provide psychiatric evaluations and medication management. Horizons Health & Wellness Center psychiatrists are compassionate and skilled physicians who understand that you may have questions and concerns about medication as a treatment modality.  

(PRP) services are offered to adult Medicaid clients who are receiving medication management and therapy but continue to struggle with daily functioning and stability. These individuals may benefit from additional structured support to build skills, improve independence, and enhance quality of life.

Minors who are receiving one or both services and continue to experience emotional, behavioral, or functional challenges, requiring added support to meet treatment goals.
