How to Get Started at Horizons Health & Wellness Center

Starting mental health services at Horizons Health & Wellness Center is simple, secure, and designed to help us connect you with the appropriate level of care. We work with adults and minors and accept , Maryland Medicaid and CareFirst BCBS.

The first step is to complete our online pre‑registration paperwork through our secure application system. This allows us to collect basic information, including insurance details, so we can verify eligibility and begin the intake process.

After submitting your pre‑registration, you will be asked to complete brief screening surveys related to anxiety and depression. These surveys are required for clinical review and help our licensed providers determine medical necessity and the most appropriate services, including those covered by insurance or Medicaid.

Once your paperwork and surveys are received, our administrative team will review your information and verify insurance or Medicaid coverage. After verification, a team member will contact you to schedule a comprehensive clinical assessment with one of our licensed behavioral health providers.

Following the assessment, our clinical team will develop an individualized treatment plan based on clinical need, insurance guidelines, and your goals for care. Services will begin once authorization requirements are met, when applicable.
